Pigment Therapy

Pigment Therapy Our coastal community offers us many wonderful opportunities to enjoy the outdoors. Long term exposure to the sun can lead to conditions such as skin cancer, premature aging, sun damage, wrinkles, and melasma.. In the past, these sun-induced forms of skin damage have been difficult to treat, but exciting new developments in laser technology have given new hope to patients with these conditions.

Correcting Pigment Irregularity – Laser therapy can be used for the treatment of skin pigment irregularity. The newer lasers that are used for these treatments have an impressive safety record. The laser wavelength used for skin lightening is attracted to the pigment so that the laser energy destroys the irregular pigment with little or no damage to the surrounding lighter skin.

New Technology - The lasers for both pigment lightening and wrinkle reduction incorporate cooling spray devices, which aid in patient comfort and skin surface protection. Minimal temporary bruising may sometimes occur, disappearing in a day or two. Treatments take about ten minutes and are scheduled every two weeks for a total of three to eight treatments.

Summary – In our experience with surface-sparing laser treatment, dramatic improvements in pigment irregularity and wrinkles have been achieved in as little as three treatments. The results have been far superior to bleaching creams, acid creams, and retinoids. In addition, the newer laser treatments are safer than deep acid peels and CO2 laser therapies. There is little or no down time involved with these forms of treatment, and the results in many patients are dramatic. Laser Hair Removal

Laser Hair Removal Since 1995 we have been performing Laser hair removal. Through the years we have had many advances in technology and now use the Candela GentleLASE Alexandrite Laser. We found this to be the most effective laser on the market, and, after using it on hundreds of patients, we are convinced that this is the best hair removal system for anyone with brown or black hair. For dark skinned patients we use the gentle Yag .These lasers are set apart from other lasers by being at an ideal wave lengths. The Candela GenteLASE Alexandrite Laser has a cooling spray that helps cool the upper layers of the patient’s skin, allowing us to deliver more energy to the follicle without damaging the surface. This cooling spray is crucial in obtaining optimal results. The Candela GentleLASE (which was upgraded to the GentleLASE Plus in January of 2000) is the only hair removal laser with this cryogenic spray for skin protection. This laser was FDA approved in 1999 and we have found that it results in an excellent permanent hair reduction.

How does the Candela GentleLASE Plus compare to other forms of hair removal? – Traditional hair removal techniques such as shaving, plucking, and waxing provide only temporary relief. Until now the only semi-permanent way to remove hair has been by electrolysis, which can be painful, time consuming, and is limited to small cosmetic areas. Scarring, allergic reactions, and ingrown hairs are side effects of all of these hair removal methods. The Candela GentleLASE Plus safely removes unwanted body hair without damaging pores and delicate structures of the skin. Facial and bikini areas are usually completed in less than 10 minutes. Legs and larger areas can take longer. Other factors may influence the treatment process and are generally best evaluated and discussed at a consultation.

What can I expect during treatment? – Depending upon the amount of hair being treated, the procedure varies from patient to patient. Generally, the treatment will include:

  1. Safety eye-wear for all people in the room to protect everyone from the intense laser light.
  2. A small hand-piece will be used with a guide that touches the skin to deliver the laser light. A light spray of coolant to the skin will be felt before each laser pulse.
  3. Most patients experience only a mild discomfort at the treatment site because the cooling device minimizes discomfort. The degree of discomfort varies with each person depending upon his/her own pain threshold. Patients with a low pain threshold may use anesthetic cream prior to the treatment.
  4. The procedure can take as little as 10 minutes or up to an hour depending upon the size of the area being treated.
  5. All patients can go home immediately following treatment.
  6. The treated area may develop small pink or red welts within thirty minutes after treatment. This typically lasts only a few hours. Rarely, it may last several days. Avoid the sun and certain prescription medications and over the counter medications before and after treatment unless authorized by the physician. You must avoid bleaching, plucking, or waxing hair for 4 weeks prior to treatment. Topical anesthetics may also be requested. Be sure to discuss this with your physician prior to your laser treatment. If you are receiving treatment around the mouth and you have a history of herpes (fever blister), a prophylactic anti-viral therapy may be prescribed. If you are tan or have a dark complexion, a skin bleaching cream may be started 4 to 6 weeks before treatment.

The amount of treatments will vary – Hair grows in 3 cycles, anagen (growth phase), catagen (resting phase), and telogen (falling out phase). The laser is only effective on the hair in the anagen or growth phase. Approximately 30% of the hair is in the growth phase at any given time. Therefore, it is rare that one treatment would be sufficient. Repeated treatments are necessary to treat hair follicles as they re-enter the growth phase. Other factors affecting patient response are skin color, hair color, and hair texture. Most patients are satisfied with 5 treatments but some patients may require more treatments.

Facial Vein Therapy

Many people have spots on their skin that they do not like. These include: skin discoloration, enlarged blood vessels, marks left by rosacea, as well as sun damage. Now, with the revolutionary Laserscope Aura these marks can be greatly diminished, or in many cases totally removed with minimal to no visible scarring.

Vascular lesions are areas of overgrown or enlarged blood vessels. These areas vary in color from red to purple and range in size from a small spot to large areas over the body. These lesions are sometimes called telangiectases, hemangiomas, or port wine stains. The lesions are sometimes present at birth, caused by contraceptives, hormone therapy, or the natural process of aging.

Treatment with older laser technology has left significant bruising, redness, and swelling. These unwanted side effects are greatly minimized with the Laserscope Aura. This revolutionary technology to treat facial blood vessels works with a series of laser light pulses. These pulses are absorbed by the blood and blood vessels, causing the blood to heat and coagulate. The blood vessel walls are then unsupported and collapsed. Over the next several months the body gradually dissolves away these unwanted blood vessels. With the vessels gone, the treated area returns to a natural skin color. In many cases, one treatment often produces significant results.

A treatment with the Laserscope Aura is performed in our office. This procedure may take anywhere from a few seconds for a small blood vessel to 45 minutes for a very large area. Treatment consists of placing a small hand-piece or scanner against the surface of the skin and activating the laser. There is minimal discomfort during treatment so there is no need for anesthesia. Because of the brightness of the laser light, all patients are asked to wear eye protection. After laser treatment you will experience redness in the treated area which will go away within a few days. Side effects are uncommon and usually minimal.

Certain prescriptions, all over the counter drugs, vitamins, and herbs should be avoided one day prior to treatment as well as the day of treatment. Sun exposure should be limited before and after treatment. Sunscreens should be applied daily. Before a laser procedure is performed, a thorough consultation with us is necessary to determine the nature of the condition to be treated. We come up with a treatment plan together, answer any questions that you may have, and give an idea of the results which may be expected. The charge is based upon the complexity of the procedure. restylane-juvederm facial injectablesRestylaneRestylane/Juvederm – Restylane and Juvederm are very similar products that are both used to treat the nasolabial folds and lip enhancement. The nasolabial folds are the facial wrinkles that follow the lines of the nose down and around the mouth. They are also known as “marionette lines.” Both Restylane and Juvederm are a naturally hydrating substances in gel form called hyaluronic acid which is depleted in the body with age. This procedure is performed by injecting the area with either Restylane or Juvederm using a small needle. This procedure is usually done in about 10 to 15 minutes. Patients usually love the look and the results often last for a year, sometimes longer.

Dysport – Dysport is a facial injectable used to temporarily treat lines and wrinkles that form between the eyebrows. By injecting Dysport, the muscles that cause what are commonly known as “frown lines” are temporarily unable to contract resulting in a smoother appearance between the eyebrows. Patients who use Dysport have treatments done every three months.


Microdermabrasion is a superficial technique in skin resurfacing that treats acne, rough and uneven skin, acne scars, blemishes, fine lines, age spots, wrinkles and uneven pigment.

Short History – Mattioli Engineering of Florence, Italy designed the Microdermabrasion system. Based on innovative design and invaluable experience gained over 30 years, this system can offer therapy to achieve a smooth, glowing skin for everyone. The simple design of the machine has been specifically aimed at the therapist who is highly qualified to carry out a wide range of treatments in the clinic on a routine basis, both safely and effectively. The system is widely used in Europe and has received FDA clearance in the United States. There are 2 versions of most microdermabrasion machines. One is created for use in spas or salons. The other is specifically designed for use in physicians’ offices by medically trained personnel. With the physician’s office model used at Atlantic Dermatology Associates, P.A., the patient can attain better results. Often we combine the microdermabrasion treatment with a chemical peel to achieve better results.

About the Treatment - Microdermabrasion is a technique in skin resurfacing. Its effectiveness allows us to treat a wide variety of skin conditions. The microdermabrasion machine projects a jet of fine crystals, which is blown, then vacuumed across the skin’s surface, removing the dead and damaged skin cells. The result will leave skin softer, smoother, healthier, and more youthful looking. We recommend glycolic acid peels in conjunction with microdermabrasion to attain optimal results. We analyze your skin condition and choose the most suitable treatment plan.

What To Expect After Treatment – The skin may look and feel as if it was exposed to the sun or wind immediately after treatment. Some skin may peel or flake. The fresh, newly exposed skin will be delicate. To ensure that the area is kept clean, moisturized, and protected from the sun patients should use a mild cleanser and a moisturizing sunblock. Make-up may be applied 2-3 hours after the procedure. The greatest benefit is that the patient will leave the office with fresh, rejuvenated skin. The treatment is mild enough to be done at a workday lunch break.

Vein Therapy

What are spider veins? - Spider veins are small blood vessels that have a red or bluish discoloration appearing frequently on the legs. These may be short, long, or come up in a “sunburst” pattern. Some people with these unwanted blood vessels of the legs even have discomfort from the veins. Spider veins carry blood but their superficial blood supply is totally unnecessary and they can be eliminated. At Atlantic Dermatology Associates, P.A., even though we have two lasers which are FDA approved for blood vessels, we find that with most patients sclerotherapy works best on the legs. Sclerotherapy is the injection of concentrated saline (salt) solution into blood vessels. The solution causes the vessels to disappear or become much smaller. Most patients experience a very significant improvement after sclerotherapy, although in some cases it may take several treatments.

What causes spider veins? – The cause of spider veins is unknown. We do know that, for genetic reasons, some people tend to have more spider veins than others. They occur in both men and women. Hormones may play a role in spider veins as well as tight clothing and sun exposure. Standing on hard floors for long periods of time may also cause spider veins. Spider veins usually cannot be prevented, but wearing support hose may help somewhat. Exercising and sun avoidance may also be helpful.

How long has sclerotherapy been around? – Sclerotherapy of spider veins on the legs has been used since 1930. The solution irritates the lining of the blood vessels causing them to swell, become irritated, shut down, and then the body gradually dissolves these veins.

Can I be treated with sclerotherapy? – Most patients can be treated with sclerotherapy. If there is a history of blood clots in the legs, the physician should be notified. In patients with a history of blood clots the procedure may still be done but with more caution. All skin types and all skin colors respond well to sclerotherapy.

What do I do after the treatments? – Slight swelling and bruising can be expected immediately after treatment. We recommend that all patients wear support hose for at least 3 days. Patients can shower/bathe and resume all normal activity. Exercise, such as walking, is encouraged; however, aerobics or jogging should be avoided during this time. The sun is your worst enemy during the “healing phase” and we strongly suggest that the patients wear long pants and avoid sun for up to 3 months after treatment.

Chemical Peels

Chemical Peels Salicylic and Glycolic acid peels are performed to minimize sun damage. Chemical peels help with skin rejuvenation, fine lines, wrinkles, and acne scarring. Often times people use chemical peels along with microderm abrasion to not only help with sun damage and acne, but to also even out skin tone. This treatment normally takes about 30 minutes.

Tattoo Removal

Tattoo Removal uses a specific laser that removes the colored pigment from the skin. Usually, tattoo removal is completed in 6 to 8 treatments each of which are about 6 to 8 weeks apart. However, depending on the color and the type of ink used, more treatments may be needed. The actual treatment itself is 1 minute with the laser.


Many patients complain about wrinkled, sagging skin or bags under the eyes. This is a non-invasive technique, which involves an electric cautery, to tighten the skin under the eyes. This treatment is usually performed every two weeks for a total of 8 to 10 treatments. The actual treatment itself is only 1 minute.

Ear Lobe Repair

Ear Lobe Repair is used to correct over stretched or damaged holes in the earlobes due to wearing heavy earrings or ripping. This treatment lasts about 15 minutes per earlobe.
